To apply to Saint Anthony School please complete the following forms:
- Application and a $30.00 fee
- Birth Certificate – copy
- Baptismal Certificate – copy
- Teacher references from previous school (1) for Preschool-K (1) for 1-3rd (2) 4-8th
- Copies of latest Standardized Tests
- Previous year’s final Report Card and current Report Card
- Welcome Packet Checklist
Once you have received a letter of acceptance to the school, you will be mailed all forms and must submit the signed Tuition Agreement Contract and applicable Fees to reserve your child’s place. To learn more about the admissions process please visit the admissions page.

Enrollment/Re-enrollment Forms: To be submitted annually
- Signed Tuition Agreement Contract and paid Registration Fee Non-Refundable and Tuition Refund Plan (TRP) required for monthly and semi-annual payments.
- Form l4 Health Record/ vaccinations noted and updated /TB clearance – Obtain form from Pediatrician or school office
- Early Childhood Pre-K Health Record Supplement
- Emergency Contact Form information for school
- Parental Permission Form: field trips, website, publicity
- Parent Acknowledgement Form: Acknowledgement of reading and agreement to the information in the school handbook and Volunteer Code of Conduct.
